Klono-Neuratemazepam

Pharmacology - Hypnotic, alchemic

    Klono-Neuratemazepam is an active benzodiazepine with hypnotic and nonconvulsant properties, run through a specific process of alchemy in the laboratories of Memphis-Hemingway Sanitarium. In sleep laboratory studies, it decreased the number of nightly awakenings in the first five hours of slumber. No tests have been conducted yet on withdrawal symptoms. Klono-neuratemazepam decreased Stage 3 and 4 sleep, accompanied by a compensatory increase in Stage 2 and R.E.M. sleep.
    Orally administered klono-neuratemazepam is well-absorbed in humanoid creatures with neurological systems the same as or similar to human. It has a half-life of about eight hours in plasma (with considerable inter-individual variability). On regular dosing, steady state is reached within four days with excretion of the drug mainly in the urine in the form of the inactive O-conjugate metabolite.


Indications

    Klono-neuratemazepam is a hypnotic agent useful in the short-term management of a specific insomnia afflicting beings with an overstimulated brainstem only (psychics). It has no effect, however, in shortening the time taken by patients to fall asleep.
    Efficacy has not been established in human children under 3 years of age or the equivalent development in similar beings, and pregnant females. In some cases, dosage adjustment may establish efficacy. As with other hypnotics, klono-neuratemazepam is not indicated for prolonged administration.


Contraindications

    Do not administer to non-psychics and those demonstrating hypersensitivity to benzodiazepines, significant liver disease, or who report a sensitivity to magic-imbued products.


Warnings

    Occupational Hazards: Since temazepam has a hypnotic effect, patients should be warned against driving, operating dangerous machinery or engaging in other activities requiring mental alertness and physical coordination within at least ten hours of taking the drug.
    Physical and psychological dependence: As with other benzodiazepines, klono-neuratemazepam should be administered with great caution to individuals prone to drug abuse. Caution should be observed in all patients whose histories suggest that they may have potential for psychological dependence. It is theorized that withdrawal symptoms which might occur after prolonged use of alchemic benzodiazepines are similar to those manifested by patients with excessive anxiety and may appear to justify continuation of the drug.
    Potentiation of drug effects: Klono-neuratemazepam may potentiate the effects of other CNS depressant drugs such as alcohol, barbituates, nobarbituate hypnotics, antihistamines, narcotics, and antipsychotic and antidepressant drugs. Therefore, different benzodiazepines should usually not be used simultaneously and careful consideration should be given if any of the above will be a likely combination. Advise patients against such combinations because of the potentiation of effects that might occur.
    Pregnancy: If the drug is prescribed to a woman of child-bearing potential, she should be warned to consult her physician regarding discontinuation of the drug if she intends to become or suspects that she is pregnant.
    Lactation: Since the drug has similar properties to other benzodiazepines, it is probably excreted in human milk. Nursing mothers should be advised not to breast-feed non-psychic children while taking klono-neuratemazepam.
    Psionics: In laboratory studies, there is a marked decrease in psionic proficiency during the use of this medication, for most common humanoid species. Classification Minor and Major psychics experienced random inability to manifest psionic effects of simple exercises. NOTE: While it is acceptable to comment on this to the patient, no elaboration is available as yet, and causing further anxiety and the possible refusal of the drug is not advised.


Precautions

    Patients with emotional disorders: Klono-neuratemazepam should be used with caution in patients with symptoms of depression or evidence of latent depression, particularly when suicidal tendencies may be present and protective measures may be necessary.
    Geriatrics and debilitated patients: These patients, or those with organix brain syndrome, are prone to CNS depression after even low doses of benzodiazepines and may experience paradoxical reactions to these drugs. Therefore, klono-neuratemazepam should be used in these patients only in the lowest possible dose.
    General: Klono-neuratemazepam is metabolized in the liver and is primarily excreted by the kidney. Hence, caution should be exercised in administration of the drug to patients who might have impaired hepatic and/or renal function.


Adverse Effects

    The most common observed adverse reactions reported after administration of klono-neuratemazepam are dizziness, vertigo, confusion, shortness of breath, and some marked decrease in psychic proficiency that is assumed temporary. Less common are: hyperactivity, euphoria, blurred vision, and skin rash. Paradoxical reactions such as excitement, stimulation, and hallucinations are observed infrequently.
    Miscellaneous effects: palpitations, coated tongue, dehydration, fever, weight gain or loss, and changes in libido


Overdose

    Symptoms: Manifestations of acute overdosage of klono-neuratemazepam can be expected to include somnolence, confusion, and reduced reflexes. Large overdoses result in respiratory depression, hypotension and finally coma.
    Treatment: If the patient is conscious, induce vomiting mechanically or with emetics. If excitation occurs, barbituates should not be used. It should be borne in mind that multiple agents may have been ingested.


Dosage

    An appropriate hypnotic dose should produce the desired effects while avoiding oversedation and impairment of performance after around ten hours.
    Adult Humans and equivalent age humanoid species: 15mg before retiring.
    Children above 4 years (and equivalent developmental stage species: 5-7mg (half a tablet) before retiring.
    Eldery and debilitated patients: 10mg before retiring (see Precautions).
    Klono-neuratemazepam is intended only for short-term use and, therefore, should not be prescribed in quantities exceeding those required for this initial cycle of administration (one week). Prescriptions should not be renewed without further assessment of the patient's needs.


Supplied

    Each cylindrical, biplane, scored tablet, edges bevelled, imprinted B-15, contains: klono-temazepam 15mg. Also contains lactose. Packets of 7.
